1. Purpose

By signing this form, you agree to allow Meta Mentor to use your submitted materials from the Beta Program — including, but not limited to, progress photos, testimonials, product reviews, and feedback — for marketing, promotional, and research purposes.

2. Materials Covered

This consent applies to:

• •Progress Photos: Any before/after or in-progress photos submitted during or after the Beta Program.
• •Product Reviews & Testimonials: Written or recorded feedback you provide about Meta Mentor.
• •Program-Generated Data: Summarized results (e.g., percentage of progress, improvements) that do not contain sensitive personal information.

3. How Materials May Be Used

You grant Meta Mentor the right to:

• •Publish your materials on our website, app, marketing campaigns, advertisements, and social media platforms.
• •Edit, crop, or adapt materials as needed for promotional use (without altering the meaning of your feedback).
• •Display materials without geographic or time limitation.

4. Privacy & Identifying Information

• •You may request that your name or identity remain anonymous (e.g., using only first name or initials).
• •Personal contact information will never be shared publicly.
• •If anonymity is requested, identifying features (such as your face) may be blurred upon request.

5. Voluntary Participation & Revocation

• •Your participation is voluntary.
• •You may withdraw consent in writing at any time before publication.
• •Once materials are published, withdrawal will only apply to future use.

6. No Compensation

You understand that participation in the Beta Program and granting this release is voluntary, and you will not receive financial compensation for the use of your materials.
